29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about September 9, 2019, the employer failed to guard the Bay 5 Automatic Feeder, thereby exposing employees to hazards created by ingoing nip points, pinch points, and rotating parts. An employee sustained an amputation of the right thumb while attempting to advance a short bar through the automatic feeder's cone into the polishing machine.
