1926.502(i) Covers. Covers for holes in floors, roofs, and other walking/working surfaces shall meet the following requirements: 29 CFR 1926.502(i)(2): All other covers shall be capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. a. On or about September 9, 2019, located on a temporary platform at the 4th floor elevator shaft, the employer did not ensure that employees were protected from falls through two floor holes on the left and right ends of the platform by ensuring that floor hole covers were capable of supporting, without failure, at least twice the weight of employees, thereby exposing employees to a fall hazard of approximately 40 feet. b. On or about September 11, 2019, located on a temporary platform at the 4th floor elevator shaft, the employer did not ensure that employees were protected from falls through two floor holes on the left and right ends of the platform by ensuring that floor hole covers were capable of supporting, without failure, at least twice the weight of employees, thereby exposing employees to a fall hazard of approximately 40 feet. On September 11, 2019, employees were working from a temporary platform located in an elevator shaft. The employee stepped onto an unsupported, unsecured and unmarked floor hole cover on the left end of the platform which caused the employee to fall approximately 40 feet resulting in death.

On or about September 9, 2019, located on a temporary platform at the 4th floor elevator shaft, the employer did not ensure that employees were protected from falls through two floor holes on the left and right ends of the platform by ensuring that floor hole covers were secured to prevent accidental displacement, thereby exposing employees to a fall hazard of approximately 40 feet. b. On or about September 11, 2019, located on a temporary platform at the 4th floor elevator shaft, the employer did not ensure that employees were protected from falls through two floor holes on the left and right ends of the platform by ensuring that floor hole covers were secured to prevent accidental displacement, thereby exposing employees to a fall hazard of approximately 40 feet. On September 11, 2019, employees were working from a temporary platform located in an elevator shaft. The employee stepped onto an unsupported, unsecured and unmarked floor hole cover on the left end of the platform which caused the employee to fall approximately 40 feet resulting in death.
