29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On September 6, 2019 at approximately 1:30pm, an employee operating a stand up forklift was hospitalized due to a work related accident. The hospitalization was not reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration. The hospitalization was reported by online on September 9, 2019 at approximately 10:36am.
