29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about 9/12/2019, near 5907 Springdale Road in Cincinnati, OH, while locating a water pipe an employee entered a trench with a measured depth of 4'8" without utilizing a ladder or other safe means of egress. The employee entered the trench by stepping into a section with a break in elevation measured to be 3'8".
