(a) On or about 3/2/2020, the employer failed to submit documentation and certification of corrective action for citation 1, item 1 issued on 10/8/2019 with an abatement due date of 10/21/2019.  (b) On or about 3/2/2020, the employer failed to submit documentation and certification of corrective action for citation 1, item 2 issued on 10/8/2019 with an abatement due date of 10/21/2019.  (c) On or about 3/2/2020, the employer failed to submit documentation and certification of corrective action for citation 1, item 3 issued on 10/8/2019 with an abatement due date of 10/21/2019.
