Location:  a) At 2250 59th Street, Brooklyn, NY 11204:    On or about 09/12/2019, employees on the jobsite used a stairway to access the cellar level of the jobsite. The employees had to walk over a hole between the ground level and the stair top step to access the stairway. The employees were exposed to falling approximately 12 feet to the lower level. The employees were not protected by the use of guardrails, covers, or personal fall arrest systems.
