29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Location: a) At 2250 59th Street, Brooklyn, NY 11204: On or about 09/12/2019, employees on the jobsite used a stairway to access the cellar level of the jobsite. The employees had to walk over a hole between the ground level and the stair top step to access the stairway. The employees were exposed to falling approximately 12 feet to the lower level. The employees were not protected by the use of guardrails, covers, or personal fall arrest systems.
