29 CFR 1926.201(a) / MUTCD Section 6E.02: Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices for Streets and Highways (MUTCD), 2009 Edition, December 2009 (including Revision 1 dated May 2012 and Revision 2 dated May 2012), in that for daytime and nighttime activity, flaggers shall wear high-visibility safety apparels that meets the Performance Class 2 or 3 requirements of the ANSI/ISEA 107-20004 publication.      Location:  a) On the northeast exterior of the jobsite at 2250 59th Street, Brooklyn, NY 11204:     On or about 09/12/2019, a flagger did not wear a high-visibility safety apparel, and was exposed to struck by hazard.

The path to ground was not continuous because the ground pin of the extension cord was missing.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  Location: a) At 2250 59th Street, Brooklyn, NY 11204:  On or about 09/12/2019, employees on the jobsite used a stairway to access the cellar level of the jobsite. The employee had to walk over a hole between the ground level and the stair top step to access the stairway. The employees were exposed to falling approximately 12 feet to the lower level. The employees were not protected by the use of guardrails, covers, or personal fall arrest systems.
