29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Scaffold North Stair Tower: Employees were exposed to a fall hazards of approximately 6 feet while engaged in masonry activities from a welded frame scaffold that did not have a proper means of access, on or about 6/20/19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Scaffold North Stair Tower: Employees were exposed to a fall hazards of approximately 12 feet while engaged in masonry activities from a welded frame scaffold utilizing an extension ladder that only extended 6" above the platform, on or about 6/20/19.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    a) Scaffold North Stair Tower: Personal Fall Arrest Systems being used by employees engaged in masonry activity from a 52 feet high welded frame scaffold platform were anchored on the cross brace pins, on or about 6/20/19.
