29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: 66 Sumner St, Milford, MA: As of September 13, 2019, the employer had not instituted energy control procedures, training, or periodic inspections to ensure outside maintenance contractors and the employer's employees were not exposed to the hazards of uncontrolled energy when performing set up, maintenance, and servicing on machines including but not limited to the Roto-Die Hydraulic Bender machine.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): 66 Sumner St, Milford, MA: On September 13, 2019 and at times prior, employees who operated the Roto-Die Hydraulic Bender machine were exposed to hazards of the incompletely guarded bending area between the upper and lower dies. On September 4, 2019, an employee operating the Roto-Die bender machine received hand crushing injuries in the unguarded area.
