29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  Production area- On or about September 16, 2019 and at times prior, an employee was exposed to a health hazard in that, the employee was allowed to wear a filtering face piece type respirator (disposable dust mask) and was not given the information regarding the use and limitations of dust mask contained in Appendix D of the respirator standard. The dust mask was used on a voluntary basis.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met:  (a) Production area- On or about September 16, 2019 and at times prior, employees were exposed to a health hazard in that, the employer failed to establish a written hazard communication program for employees exposed to hazardous substances such as, but not limited to, a laminate adhesive containing toluene and hexane, machine hydraulic fluids, and propane.
