29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to employees. Such facilities were not in near proximity to the worksite and were not equipped as to enable employees to remove such substances:  a)  On or about September 10, 2019, at the job-site, no hand washing facilities were available in the work area was available for subcontracted employees performing stucco work handling masonry concrete which contains Portland cement, exposing them to chemical burns.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) On or about September 10, 2019 and prior to that time, at the south side of the commercial building, the supported scaffold was erected, disassemble or altered without supervision and direction of a qualified competent person.  Subcontracted employees were exposed to fall and struck-by hazards, in that, the scaffold system had deficiencies in the fall protection and was disassembled or altered while workers where working from it on lower levels.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a) On or about September 10, 2019, at the south side of the commercial building, the west end side of the fifth, sixth and seventh levels of the supported scaffold used by subcontracted employees to plaster stucco on the wall had no guardrail system installed, exposing to a 32 foot, 38.4 foot and 44.8 foot fall hazard respectively.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  a) On September 10, 2019 and prior to that time, at the south side of the commercial building, the supported scaffold was erected, disassemble and altered by employees who were not trained by a qualified competent person.  The scaffold was disassemble while workers where working from it on lower levels and portions of the fall protection were removed while subcontracted workers were applying stucco, exposing them to fall and struck-by hazards.
