29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a) 920 Back Road, Berwick, PA: The employer did not follow the manufacturer's instructions when installing anchors for a fall arrest system in that anchors meant to be installed on a sheathed wood truss structure with twelve #12x14 two inch screws were improperly installed in a metal truss roof with only four to six #12 1 1/2 inch screws, on or about September 10, 2019.  CORRECTED DURING INSPECTION CERTIFICATION NOT REQUIRED

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) 960 Back Road, Berwick, PA: Employees were performing commercial roofing activities with a personal fall arrest system that included a fifty (50) foot lifeline anchored within two (2) feet of a twenty-four (24) foot fall hazard, exposing employees to a free fall hazard, on or about September 16, 2019.
