Site - On September 13, 2019 an employee was seriously injured when he fell approximately 7 feet through an unguarded opening in the floor to the concrete basement slab below. b. Site - An employee was exposed to falls of approximately 10 feet when he was observed walking along the unfished second story of the building without the use of fall protection.
