ADT Construction, Inc. on or about September 17, 2019, at the address of 5055 Liberty Ave., Vermilion, Ohio:  The employer did not have a plan or program to identify job site hazards for employees to maintain a safe working environment.  Such a program would allow the employer to identify potential fall and electrical hazards during job site operations, decreasing the chance of employee injury.
