29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about September 11, 2019 at the worksite located at 1318 Browns Bridge Road in Gainesville, Georgia, employees utilizing a Genie S - 40 aerial lift were exposed to a fall hazard of approximately 10 feet, when the employer failed to ensure that each employee in the aerial lift was utilizing a body belt with lanyard attached to the boom or basket of the aerial lift. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about September 11, 2019 at the worksite located at 1318 Browns Bridge Road in Gainesville, Georgia, an employee mixing grout material was exposed to health hazards when the employer failed to properly train the employee on the hazards associated with the chemicals the employee was using. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
