29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. On or about 17 September 2019, and at times prior thereto, a roofer performing roofing activities on a residence under construction was not wearing head protection while under the roof eave, exposing the roofer to the hazard of being struck by the eave or materials, tools, and equipment upon descending from the roof.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about 17 September 2019, and at times prior thereto, a roofer was using a pneumatic nail gun to apply new shingles to the roof of a residence under construction without the use of eye protection, exposing the roofer to the hazard of being struck by flying debris or projectiles.
