29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used and scaffold cross bracing was permitted to be used:      a) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about September 17, 2019, during masonry work, the employer did not ensure that appropriate safe access/egress was provided from the top of the 2nd scaffold frame to lower areas such as outriggers or the 1st scaffold frame.    b) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about September 17, 2019, during masonry work, the employer did not ensure that appropriate safe access/egress was provided from the top of 1st scaffold frame to ground level in that they used a step ladder set up approximately 10 inches or so below the platform on the 1st level.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:      Instances including, but not limited to, the following:    a) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about September 17, 2019, during masonry work, the employer did not ensure that appropriate fall protection systems/devices were in use along the rear side of the top of the 2nd scaffold frame, exposing an employee to a fall hazard of approximately 13 feet in that no form of fall protection was in use in one frame and in another only a mud box was present.      b) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about September 17, 2019, during masonry work, the employer did not ensure that appropriate fall protection systems/devices were in use along the end of the top of the 2nd scaffold frame, exposing an employee to a fall hazard of approximately 13 feet in that no form of fall protection was in use.     c) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about September 17, 2019, during masonry work, the employer did not ensure that appropriate fall protection systems/devices were in use along the ends of the outrigger on the 2nd scaffold frame, exposing an employee to a fall hazard of approximately 10 feet, 2 inches in that no form of fall protection was in use.    d) Job site, 7601 McKnight Road, Pittsburgh, PA: On or about September 17, 2019, during masonry work, the employer did not ensure that appropriate fall protection systems/devices were in use along the sides of the top of the 2nd scaffold frames, in that cross bracing was used as top rail at height of approximately 48 inches above the platform and midrails were not installed, exposing an employee to a fall hazard of approximately 13 feet.
