29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section. a) Jobsite: 724 Chestnut St, Philadelphia, PA, 2nd and 3rd floors - Employees performing housekeeping activities from the second floor of the commercial/residential building, in proximity to open and unprotected floor edges, approximately 14' 9" and 24' 9" above ground level, were exposed to fall hazards without the use of a fall protection system. Noted on or about 9/11/19.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a) Jobsite: 724 Chestnut St, Philadelphia, PA, 2nd floor concrete block shaft opening - The guardrail system protecting the concrete block shaft opening was missing a midrail exposing employees conducing renovation activities in proximity to the shaft opening to a 14' 9" fall hazard. Observed on or about 9/11/19
