On or about August 7, 2019, employees were exposed to falling approximately 12 feet to the ground below when stepping off of the catwalk to load feed into trucks and fall protection systems were not used.  b:     On or about September 4, 2019, employees working in the Mixing Area were exposed to falling approximately 40 feet to the ground below because the manual hand-powered elevator did not have fall protection provided when operated.
