29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: On or about September 17, 2019, the employer failed to ensure that employees in the process of replacing a 6 inch water main at a site located at 15381 Royalton Rd. in Strongsville, OH, were protected from excavated materials (the spoil pile) falling back into the trench by placing the material at least 2 feet from around the edge of the trench or by the use of a retaining device.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about September 17, 2019, at a jobsite located at 15381 Royalton Rd. in Strongsville, Ohio; the employer did not ensure that adequate cave-in protection was used to protect employees working inside of a trench which varied in depths of 9.5 to 11 feet, during the installation of a 6 inch water main. Additionally the ladder used for egress was located outside of the protective system thus increasing the potential of exposure to hazards created by the inadequately protected trench. Sals Heating and Cooling Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.652(a)(1), which was contained in OSHA inspection number 1081901, citation number 1, item number 2 and was affirmed as a final order on September 15, 2015, with respect to a workplace located at 8365 Royalton Rd., North Royalton, OH 44133.
