Research and Development: On or about September 18, 2019, and at times prior, the employer exposed employees to electrical hazards, in that an Amazon Basics brand relocatable power tap, listed and labeled by Underwriter's Laboratories, was cable tied to a metal frame and was plugged into an extension cord rather than being plugged directly into a branch circuit receptacle.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a. Research and Development: On or about September 18, 2019, and at times prior, the employer exposed employees to electrical hazards, in that there were four, 7/8 inch diameter unused openings at a height of 4 feet, 2-3/8 inches above the walking/working surface on a PLC (programmable logic controller) control panel located at the bottom hemming machine prototype development station.
