The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

Roofers were installing shingles on a roof that varied from 10-22 feet with the employees working at different levels. The employees were not utilizing proper eye, head or fall protection which exposed them to falls up to 22 feet and struck by hazards to the eyes.

Roofers were installing roofing shingles utilizing a pneumatic nail gun set at 90 psi and were not utilizing eye protection which exposed the them to struck by hazards to the eyes.

The eave of the roof measured 10 feet above the ground level where the ladder extended 20 inches beyond. The following condition exposed employees to falls up to 10 feet.
