Byler Bros Construction LLC., on or about September 17, 2019, at the address of 258 South Gamble Street, Shelby, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during roofing operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Byler Bros Construction LLC., on or about September 17, 2019, at the address of 258 South Gamble Street, Shelby, OH: The employer did not ensure that a fall protection training program was in place for employees that were exposed to fall hazards over six feet in residential roofing. The training program would support employees in identifying hazards associated with roofing, common hazards could include falls from heights greater than 6 feet.

Byler Bros Construction LLC., on or about September 17, 2019, at the address of 258 South Gamble Street, Shelby, OH: One employee was observed performing roofing operations to a residential structure. The employer did not ensure that the employee was protected from falls while working above six feet from the level below. The employee was observed installing batten strips to the roof of the residential structure.
