Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job operations.

Ernesto MTZ Construction LLC. on or about September 18, 2019 at the address of 1 Main Street, Toledo, OH: The employer failed to ensure employees utilizing scaffolding were properly trained by a competent person to recognize and avoid hazards specifically associated with this type of scaffolding, such as fall protection requirements while working from an elevated work platform of this nature.

Ernesto MTZ Construction LLC. on or about September 18, 2019 at the address of 1 Main Street, Toledo OH: The employer did not ensure that employees were protected from a fall while working from masonry walk through scaffolding. The employees were exposed to a potential 27 foot fall to the level below, while laying brick on the side of the commercial structure.
