On or about September 18, 2019, employees were exposed to a cave in while performing excavation work at a commercial building located at 1601 Portage Trail, Cuyahoga Falls, Ohio 44223. The employees were in a 6.5 feet deep trench and the employer did not ensure cave in protection was in place.
