29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) On or about September 19, 2019, at 425 Harrison Ave., New Orleans, LA, the employer failed to ensure a ladder was attached to a scaffold employees accessed and egressed to perform bricklaying operations.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity). a) On or about September 19, 2019, at 425 Harrison Ave., New Orleans, LA, the employer failed to ensure employees working from a scaffold was protected by a guardrail system or wore fall protection.
