An employee engaged in warehousing activities was injured and lost time (days away) from work due to the injury which was not recorded in the OSHA 300 log.    Violation occurred on or about September 20, 2019    c. 1000 Riverside Drive, Keasbey, NJ    The employer did not record an injury on the OSHA 300 log within 7 days of receiving notification the injury was recordable.  An employee engaged in warehousing activities was received injuries after being struck by a forklift and lost time (days away) from work due to the injury, which was not recorded in the OSHA 300 log.    Violation occurred on or about September 20, 2019    d. 1000 Riverside Drive, Keasbey, NJ    The employer did not record an injury on the OSHA 300 log within 7 days of receiving notification that  the injury was recordable.  An employee engaged in warehousing activities was injured on July 24, 2019 and lost time due to the injury which was not recorded in the OSHA 300 log. Employees engaged in the moving of over the road trailers operated Ottawa T2 Switcher trucks were not certified by the employer prior to being authorized to operate the equipment.
