29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:       On or about September 20, 2019, although the employer established a program for the control of hazardous energy, it was deficient in that; the scope and purpose, and a specific statement of the intended use were missing. The employer also failed to identify the hazardous energy sources which needed to be render safe, therefore specificity of written procedures for machinery such as, but not limited to, the door router machinery was missing.  Energy sources used at the facility include, but are not limited to; electricity, pneumatic energy and gravity.

1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.      On or about September 20, 2019, the employer did not ensure that various equipment or machinery used at the site was effectively guarded.  The unguarded equipment includes; but is not limited to, the following:     a. The three (3) punch presses located in the Framing department were not adequately guarded. The affixed guarding had a gap of approximately 1-1.8125 inches, and did not effectively protect the operator from potential hazards created by the moving/rotating part(s).      b. The ten (10) punch presses that were a part of the Patio Door line were not adequately guarded. The affixed guarding had gaps that varied between 1-5 1/8 inches. Operators were potentially exposed to hazards created by the struck by, caught-by or moving/rotating part(s).      c. The one (1) single pallet wrapping machine located at the Patio Door line was unguarded and did not prevent operators from having any part of their bodies in the danger zone during operation. Employees tasked with operating the machinery are exposed to potential rotating and/or caught by hazards.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed:    On and before September 20, 2019, the lower exposed portion of the company's Delta (900 and Shop Master) radial saws was not equipped with a device that automatically adjust itself to the thickness of the stock. Employees who used the saws when cutting headers for door frames or other wood working activities were not protected from inadvertently contacting the rotating saw blade.
