29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) On or about September 20, 2019, at the above addressed jobsite, employees performing masonry operations from a tubular welded framed scaffolding approximately twelve (12) feet to fifteen (15) feet above ground, were exposed to injuries from falls due to the lack of a complete guardrail system. The scaffolding was erected without endrails, midrails and guardrails where employees were exposed to falls.
