29 CFR 1917.125: Ditches, pits, excavations and surfaces in poor repair were not guarded by readily visible barricades, rails or other equally effective means.: Conley Terminal, Container Maintenance Dept.: The employer did not cover or place barricades around a pot hole. An employee was injured when the Hyster machine used to transport containers tipped due to hitting the pot hole.
