29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings:    On or about September 23, 2019, employees were exposed to two sets of steps that were not uniform at the 52 inch slitter line. The east side steps had three risers measuring 10 inches, 9 inches, 9 inches and the north steps had three risers measuring 8 inches, 9 inches and 10 inches.  The employer did not ensure stairs had uniform riser heights.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about September 23, 2019, the employer did not ensure the following machinery was properly safeguarded.    1. 20 inch slitter line, drive side un-coiler, leveler, pinch rolls, shear, stringer baler and re-coiler.       2. 24 inch slitter line, drive side un-coiler, leveler, pinch rolls, shear, stringer baler and re-coiler.     3. 52 inch slitter line, operator side and drive side un-coiler, leveler, pinch rolls, shear, stringer baler and re-coiler.     4. Oscillator line, operator and drive side from the un-coiler through the butt welder and leveler to the re-coiler.    5. Ski Edge area, cut to length line and bending machinery operator and drive side un-coiler, leveler, pinch roll, shear.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:     On or about September 23, 2019, there were two areas where electrical control boxes had unused openings and the employer did not ensure they were effectively closed.    1. On the north wall at the ski edge area there was an electrical disconnect switch labeled (Cut To Length Line - 240 Volts) with an unused opening at the bottom of the box.    2. On the west wall at the 24 inch slitter line re-coiler there was an electrical control box labeled (Cold Mill 220V) with two unused openings in the side of the box.

The cover for the large electrical box was ajar and not secured.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:     On or about September 23, 2019, at the northwest corner of the building there were oxygen and acetylene cylinders stored in the same expanded metal cage. There was a cutting torch cart that contained an oxygen cylinder and acetylene cylinder with hoses, gages and a hand torch along with a separate oxygen cylinder and a separate acetylene cylinder that were chained on the inside of the cage.
