29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: At this worksite, employees engaged in construction activities, utilized an extension ladder to access/egress the roof. The ladder did not extend at least three feet over the roof landing, exposing employees to fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: At this worksite, employees engaged in installing a residential tile roof were exposed to fall hazards from a height greater than six feet above ground level due to lack of fall protection, including, but not limited to, a full body harness, lanyard, tie-off location or a guardrail. Oscar Mancinas-Bonilla was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1388910, citation number 1, item number 1 and was affirmed as a final order on July 16, 2019, with respect to a workplace located at 5626 Ocean Drive, Corpus Christi, Texas 78412.
