29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  For Citatoin 1, Item 1, issued on 9/26/2019 with an abatement date of 10/24/2019, the employer did not submit documents demonstrating complete abatement of the cited violation to OSHA.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    The employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a flat roof that was less than 50 feet wide.  The employees were exposed to fall hazards of approximately ten (10) feet.  Wayne Elmore dba Southpaw was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1145500, citation number 2, item number 1 and was affirmed as a final order on 6/15/2016, with respect to a workplace located at 20 Central Blvd, Highland, IL 62249.  Wayne Elmore dba Southpaw was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1088702, citation number 1, item number 2 and was affirmed as a final order on 10/13/2015, with respect to a workplace located at 401 Sherman St., Belleville, IL 62208.
