29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) St. Francis St and N Conception St (North West Comer: Bienville Square): On or about September 23, 2019, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with work performed in a Posi-Plus Technologies 800-40-015 Vehicle Mounted Aerial Lift thus exposing their employees to hazards such as but not limited to catapult hazards and fall hazards of approximately twenty (20) feet to the asphalt below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) St. Francis St and N Conception St (North West Comer: Bienville Square): On or about September 23, 2019, the employer failed to ensure exposed employee(s) were using a body belt with lanyard attached to the approved anchor point while using a Posi-Plus 800-40-015 vehicle-mounted aerial lift to perform fiber/cable installation activities; exposing employee(s) to catapult hazards and fall hazards of approximately twenty (20) feet to the asphalt below.
