Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Randy Evans Construction located at 5063 State Route 93, Oak Hill, OH 45656:  On and preceding September 24, 2019, the controlling employer did not ensure that employees engaged in a roof tear off on a low slope roof less than 50 feet in width were protected from falls by use of a safety monitor or other means of fall protection.  Approximately 9 employees were removing shingles and sweeping a roof that was 12 feet 6 inches high with the worker designated as the safety monitor actively engaged in sweeping the roof.  The following condition exposed employees to falls of 12-13 feet.
