29 CFR 1910.134(c)(2)(i): The employer did not determine if the voluntary use of a respirator did not in itself create a hazard and did not provide the respirator users with the information contained in appendix D of this section: (Construction Reference 1926.103)(a)    (a) Worksite:  On or about 9/25/2019, the employer did not ensure that an employee who was provided with a half-mask tight-fitting respirator, was properly trained, and received the information covered in Appendix D, for the use and care of this type of respirator.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a) Worksite:  On or about 9/24/2019, the employer did not ensure that employees were protected from fall hazards at a height of approximately twenty one feet above the lower level while holding, cutting and positioning a piece of plywood near the roof's edge.    Multi Roof Maintenance, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1298429, citation number 001, item number 01 and was affirmed as a final order on 7/3/2018, with respect to a workplace located at 180 Main Street, Hackensack NJ, 07601.
