Location: 21 Stackhouse St. New Bedford, MA On or about September 23, 2019, an employee was electrocuted, when an aluminum extension ladder contacted with the induced current from overhead power lines (consisting of a 13.2 kV, 3 phase circuit).

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Location: 21 Stackhouse St. New Bedford, MA On or about September 23, 2019, employees were exposed to electrical hazards while using an extension ladder, where the employer did not instruct each employee in the procedures to be followed to minimize these hazards.
