29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about September 25, 2019, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about September 25, 2019, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.307(e)(1)(i): Horizontal belts which had both runs 7 feet (2.128 m) or less from the floor level were not guarded with a guard that extended to at least 15 inches (38.1 cm) above the belt or to a standard height specified in Table O-12:  a.  On or about September 25, 2019 at the above addressed jobsite, the belt on the air compressor was not fully enclosed or guarded against accidental contact, thereby exposing the employees to the hazards associated with lacerations.

On September 25, 2019, at the above addressed jobsite, employees performing residential roofing operations, twelve (12) feet six (6) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
