On or about September 25, 2019, the employer did not ensure working surfaces were maintained free from hazards in that pallets with broken and loose boards  were used as a walkway and work platform for employees performing abrasive blasting operations.

29 CFR 1910.94(a)(7): Abrasive blasting dust(s) were permitted to accumulate on the floor or on ledges outside of the abrasive blasting enclosure(s):    On or about September 25, 2019, in the blasting area, the employer did not ensure that abrasive blasting dust did not accumulate on the floor outside of the rotary blast cleaning table in that piles of abrasive blasting dust was observed on the floor surrounding the blasting enclosure and dust collection system.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:    On or about November 14, 2019, in the blasting area, the employer did not provide employees who voluntarily wore dust masks while performing abrasive blasting work with the information contained in Appendix D of 29 CFR 1910.134 which covers precautions employee should take with the respirator.
