29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. 299 Lexington Street, Woburn, MA On or about September 25, 2019, employees were exposed to cave-in hazards while installing an 8-inch water main in a 64-inch deep trench with straight cut earthen walls without a protective system in place.
