29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a.) Benjamin Steel Company, Inc. located at 3111 St. Johns Rd. Lima, Ohio; On or about September 25, 2019, the employer did not ensure employees were protected from amputation hazards while operating the HFA-400W AMADA horizontal metal cutting band saw located in the metal process and service area. Employees are required to work around and enter the horizontal band saw blade area to collect trapped round metal runs and are exposed to an amputation/caught between hazard in that the saw blade and the collecting tray area are not guarded. An employee left index finger was struck by the metal band saw blade causing laceration and amputation injuries to the worker's left index finger.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) Benjamin Steel Company, Inc. located at 3111 St. Johns Rd. Lima, Ohio; On or about September 25, 2019, the employer did not ensure energy control application steps to control the electrical, hydraulic, and pneumatic energy sources were implemented on the HFA-400W AMADA horizontal metal cutting band saw located in the metal process and service area prior to engaging in clearing round metal runs or during blade change activities. Employees were exposed to amputation injuries in the event of the horizontal metal band saw machine starting unexpectedly. The employer did not ensure that energy control applications steps to control the electrical energy sources were implemented prior to maintenance tasks; the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a.) Benjamin Steel Company, Inc. located at 3111 St. Johns Rd. Lima, Ohio; On or about September 25, 2019, the employer did not ensure that employees were adequately trained on the company's lockout program for the HFA-400W AMADA horizontal metal cutting band saw located in the metal process and service area. Employees did not lock out and tag out the electrical panel while clearing trapped metal rounds material or during blade changeout activities on equipment including, the HFA-400W AMADA horizontal metal cutting band saw. Employees were exposed to caught in hazards such as laceration and amputations in that the HFA-400W AMADA horizontal metal cutting band saw remained energized as employees frequently conducted metal rounds material clearing and during blade changeout.
