29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:  a) 965 Frankford Ave., Philadelphia, PA 19125 - The employer did not ensure that a fabricated frame scaffold, on which employees performed brick masonry at heights up to 26 feet, was guyed, tied, or braced to prevent scaffold collapse when the scaffold exceeded the height to base ratio of 4:1, thus exposing employees to fall from height hazards. This condition was noted on or about May 23, 2019.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): [Construction Reference: 29 CFR 1926.602(d)]  a) 965 Frankford Ave., Philadelphia, PA 19125 - The employer did not certify that an employee who was operating a JLG telehandler model 10054 powered industrial truck had been trained and evaluated, thus exposing employees to struck-by and caught-in/between hazards.  This condition was noted on or about May 23, 2019.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a)  The employer did not ensure that the scaffold platform was attached to the forks of a JLG model 10054 telehandler forklift while an employee stood on the platform at heights up to approximately 26 feet, thus exposing the employee to fall from height hazards.  This condition was noted on or about May 23, 2019.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) 965 Frankford Ave., Philadelphia, PA 19125 - The employer did not provide a means of access from the ground level or from one level of the scaffold to another by use of ladders, stair towers, or other acceptable means, where employees worked at heights up to approximately 26 feet above the lower level, thus exposing employees to fall from height hazards.  This condition was observed on or about May 23, 2019.
