On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019 , the 3rd working level of a tubular frame scaffold system, located on the North side of the building, were not fully planked or decked between the front uprights and the guardrail supports, exposing subcontractors to a 20-foot fall hazard. b. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019 , the 4th working level of a tubular frame scaffold system, located on the East side of the building, were not fully planked or decked between the front uprights and the guardrail supports, exposing Subcontractors to a 26-foot fall hazard.
