29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019 , the 3rd working level of a tubular frame scaffold system, located on the North side of the building, were not fully planked or decked between the front uprights and the guardrail supports, exposing subcontractors to a 20-foot fall hazard.  b. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019 , the 4th working level of a tubular frame scaffold system, located on the East side of the building, were not fully planked or decked between the front uprights and the guardrail supports, exposing Subcontractors to a 26-foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019, the employer exposed subcontractors to fall hazards, in that an point of access to the scaffold, such as but not limited to a hook-on ladder, was not provided for use.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart.  a. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019, the employer exposed a subcontracted employee to fall hazards greater that 20-foot, in that the cross-braces were used as a toprail, there was no midrail installed, and endpoints at each upright were more than 48 inches apart as the employee performed stucco work.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  a. On a scaffold at a commercial construction site, located at 207 Miracle Strip Parkway S. in Fort Walton Beach, Florida; On or about September 26, 2019, the employer exposed subcontractors to struck-by and fall hazards greater that 20-foot, in that cross braces were missing from scaffold platforms, which did not secure vertical members together laterally, as subcontractor's employees conducted stucco work.
