(b) On or about 09/26/2019 - Bagging Building at Rethceif Bagger Machine, employees were exposed to deflagration and fire hazards due to wood dust accumulations on the elevated surfaces such as, but not limited to, equipment surfaces, and overhead ductwork.

Mallis Wulftec Stretch Wrapper Machine, the in-going nip point and rotating parts were not guarded exposing employees to caught-in hazards.  (b) On or about 09/26/2019 - Bagging Building at Premiere Tech Bagger Machine, the guards were missing at the bag sealing operation exposing employees to struck-by and caught-in hazards.
