Broad St., Philadelphia PA 19107 - The employer did not ensure that an employee, who was acting as a signal person on a roof with an unprotected edge approximately 48 feet above the lower level, was protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, thus exposing the employee to fall from height hazards. This condition was noted on or about May 22, 2019.
