29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: (a) Colorado Mountain Builders, Inc. at 280 Pickle Point, Black Hawk, CO: On and around September 21, 2019 the controlling employer did not ensure that metal stakes securing wooden concrete forms and at the base of an approximately 8 foot tall scaffold were guarded, where employees could fall. This condition exposed employees to impalement hazards.
