29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems:  (a) Roof - On or about  September 23, 2019 the employer allowed employee to work on roof without fall protection exposing employee to fall hazards.  Skylights installed on roof, which are not capable of supporting the weight of an employee, were not covered or guarded.

Or, when performing work that was both infrequent and temporary, the employer did not use a designated area:  (a) Roof - On or about September 23, 2019 the employer allowed employee to work on roof without fall protection and did not establish a designated area exposing the employee to fall hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  (a) Residential site - On or about September 23, 2019 at approximately 9:30 a.m. an employee was hospitalized as a result of being injured while at work and the employer did not report the hospitalization to OSHA within 24 hours.  Event was reported to OSHA on September 25, 2019 at approximately 12:15 p.m.
