29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about September 25, 2019, employees operating nail guns were not provided eye protection and was being exposed to eye injuries from flying debris.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On or about September 25, 2019, employees installing roofing shingles on a 8:12 pitch roof system did not use fall protection systems while exposed to an 8-foot, 6-inch fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about September 25, 2019, left side of home, the ladder used to gain access and exit from the roof was not extended 3 feet past the landing, exposing the employee to fall hazards.
