At the worksite - On or about July 18, 2019, employees engaged in construction activities were required to work form heights greater than six feet while installing roof rafters. Employees were required to work from  the ceiling joists and from the top of a second floor wall without the use of a fall protection system. The employer failed to ensure employees were provided fall protection, thus exposing employees to a fall hazard up to approximately 20.7 feet.
