29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  Location: 724 59th Street, Brooklyn, NY 11220 - Fourth Floor  On or about: September 30, 2019  (a) The extension cord used to energize a cement mixer was missing the ground pin.  T&S Home Improvement Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.404(f)(6), which was contained in OSHA inspection number 1385008, citation number 1, item number 3 and was affirmed as a final order on June 12, 2019, with respect to a workplace located at 814 51st Street, Brooklyn, NY 11220.
